The SEO Incident Report is used to document accidents, injuries, medical situations, mental health emergencies, crime, misconduct, and off-site participant incidents.

Anyone may report an incident by emailing the Safety Committee, incident@seo-usa.org.   The SEO Incident Report, however,  should only completed by those who have completed the SEO Incident Report training. Incident reports should be completed immediately within 24 hours of the event that has occurred. This ensures that the involved individuals or witnesses can recall the details of the occurrence with clarity to fill the report form accurately.
